Click here to download testing schedule ______________________________________________ ATTENTION STUDENTS ENTERING 7TH GRADE FOR THE 2012-13 SCHOOL YEAR: To comply with the Utah State Immunization Law for 7th Graders, all students entering the 7th grade for the 2012-13 school year are required to have an additional 'Tdap' shot (Tetanus/Diphtheria/Pertussis) as well as a 'Varicella' shot (or parents signature on Pink immunization card in school student file stating student has had chickenpox disease). You may get your shots from your health care provider or from the Health Department which is sponsoring a special immunization clinic. You need to bring your immunization record with you to the clinic. _______________________________________________
